** We will be holding a Drop in Session on Thursday 18th October from 9am - 2pm at PKAVS for anyone who would like to find out more about the 'Getting it Right for Cares Across Perth & Kinross' consultation. This will be an opportunity for anyone to come along and speak to someone to find out more about this consultation or to get some help in completing their survey. It will be very informal and you can drop in anytime between 9am and 2pm. This is open to anyone who is interested. **
This feedback form is your opportunity to have your say about how best we can fit services and support around your needs as a Carer. We recognise that our services across health, social care, third and independent sectors need to reflect how they currently support people in a caring role and where necessary, amend how we do things to reflect a change in culture and practice that is supportive, flexible and empowering.
All of the feedback that we receive will be read and collated by Christine Tse on behalf of the Perth and Kinross Health and Social Care Partnership. The information you provide in the survey will be entered into an electronic database held by PKAVS, and individual responses will not be shared with third-party organisations. For more information about how PKAVS manages your person data visit: www.pkavs.org.uk/Privacy-Policy
The overall survey responses will inform the new Carers Strategy, in order for us to have the first draft ready for carers to give further feedback on at the Carers Conference on the 22nd of November we are asking for carers to give their responses by the 21st October.
If you would like us to send you out a paper version please phone the Carers Hub on 01738 567076.
Once completed please return to Christine Tse, PKAVS, North Methven Street, Perth PH1 5PP.
If you have any questions you can call or email Christine on 01738 567076 or firstname.lastname@example.org
